Extended School Day Enrollment Form

This form has been provided to your student due to credit recovery needs in order for graduation requirements to be met online.

Extended School Day will be held on the following dates and times: Monday and Wednesday from 2:00 to 3:30. Extended School Day will begin 11/7/22 and run through 4/26/23.

Please submit this form no later than Friday, November 4, 2023.
